This page covers this website only. Care you receive from us is handled separately, under the clinical records practices of the practice.
What this site collects
Only what you type into the request form and send us: your name, phone number, email address, ZIP code, when you would like a visit, which treatment you are asking about, who the visit is for, whether you have had IV therapy before, and anything you write in the last box.
The site sets no cookies, runs no analytics or advertising trackers, and has no accounts to sign in to. If you do not send the form, we do not receive anything about your visit.
Please keep medical detail off the form
The request form is an ordinary web form, not a clinical portal, and we do not treat it as one. Do not send symptoms, diagnoses, medications or any other private medical information through it. After we read your request we send a medical intake form, and that is where clinical information belongs.
What we do with it
We use it to reply to you, to ask any follow-up questions, and to schedule a visit. We contact you by the phone number or email address you give us. We do not sell your information or share it for anyone else's marketing.
Requests reach us through a third-party form delivery service. We will name that provider here before launch.
No payment happens here
No payment is taken on this website, and we do not ask for card or banking details through the request form.
Asking us about your information
To ask what we hold, correct it, or ask us to delete a request you sent, call or text 718-701-8691. We will confirm your identity before acting on the request.

If this page changes, the updated version replaces this one here.
